Revival Running Dog Walk Registration Form: 

Please read the information below before completing the registration form. 
Important Information:
  • This form is to be completed to sign up for the FlatOut Dogs Revival Running Dog Walk Training.

  • Submitting this form will notify FlatOut Dogs of your interest to join this online course. 
    Following receipt an invoice will be sent to the email provided. Payment confirms entry. 


    FlatOut Revival Running Dog Walk Training
     - Price - $225cad /4 weeks (plus any appliable taxes).


    Official Start Date: 
     - February 17th, 2025 or...
     - Any Monday following February 17th, 2025.
    *Specification of a start date will be available on the entry form. 

    Payment due dates: 
     - Entry to this training group will occur once this registration form is complete, payment is received, and then on your selected start date.  
     - Payment is due upon receipt of an invoice.
     - After a 4 week session, unless there is notification of withdraw from the group, an invoice will be sent for the next 4 weeks. 

    Refund Policy:
     - No refunds will be provided. 
     - FlatOut Dogs will only issue a refund if the Training group is cancelled for any extenuating circumstances. 

After the submission of this form, your will receive a confirmation email from FlatOut Dogs!
